Practical treatments for suicidality and treatment resistant depression in both inpatient and outpatient settings are limited. Rapid, safe, and effective treatments are urgently needed. The overall suicide rate increased 40% between 2010 and 2022. Recent research shows that ketamine is proven to be a viable option for Suicidal Ideations (SI), with a total elimination of symptoms in 58.6% of patients.
Suicidal ideation refers to thinking about or making plans for suicide. This can range from fleeting thoughts to detailed planning. Suicidal ideation is a common experience and is not necessarily a sign that someone is in imminent danger of harming themselves. However, all thoughts of self-harm should be taken seriously.
Passive suicidal ideation refers to the act of thinking about suicide without any intention of carrying it out. It is often seen as a way of coping with difficult emotions or situations and may be a sign that someone is struggling to cope with their mental health. While passive suicidal ideation is not usually harmful in and of itself, it can be a warning sign that someone is at risk of harming themselves or taking their own life.
Active suicidal ideation refers to not just thinking about committing suicide but involves planning it with the intent to die.

Annually in the U.S., more than 9.4 million adults have serious thoughts about committing suicide, with hundreds of thousands seeking treatment for suicidal ideation. Since the pandemic, we’ve noted a further increase in treatment resistant depression with suicidal ideations.
There is only one medication that is FDA approved for reducing suicidal behavior, clozapine, but can only be prescribed to patients with schizophrenia or schizoaffective disorder, and it works slowly. People struggling with suicidal ideation need a fast, safe, and proven treatment and they need it now.
Although the factors involved in suicidal ideation and behaviors in any one individual is complex and multifactorial, it has been suggested that dysregulation in glutamate neurotransmission may underlie suicidal ideation and, to date, ketamine is the most widely studied glutamate modulator for the treatment of psychiatric disorders.
A ketamine study was performed in an outpatient psychiatric clinic with 231 patients of which 133 were suicidal. Suicidality decreased in 78.9% of patients and Suicidal Ideations were eliminated in 58.6% of patients. The research was focused on the medicine ketamine alone and did not involve psychotherapy although some patients did have outpatient psychotherapy, and many were on psychopharmaceuticals.
Since the above study was published and presented in 2019, more than 350,000 people have committed suicide.

It is believed that suicidal ideation and behaviors are a result of dysregulation in glutamate neurotransmission. Ketamine is the most widely studied glutamate modulator for the treatment of psychiatric disorders, including depression, anxiety, PTSD, and suicidal ideation.
Research shows that ketamine infusions or injections administered in a therapeutic setting result in a reduction of symptoms for both depression and suicide.
The VA limits its prescription of ketamine to Spravato, a nasal spray approved for use with an oral antidepressant to treat treatment-resistant depression. The VA also states that they prescribe on a case-by-case basis.
